Under 29 CFR 1910.178(l), training combines formal instruction, practical training, and an evaluation of performance in the workplace. The employer certifies each operator's training and evaluation, and re-evaluates at least every three years. Unsafe operation, an accident or near miss, an evaluation showing unsafe operation, assignment to a different truck type, or a workplace change that affects safe operation calls for refresher training, and an evaluation that it worked. Tenet provides the training, the evaluation, and the records. Authorization to operate stays with the employer.
